Completing Your Profile & Medical Intake

A step-by-step guide to finishing your registration and providing the required information
Completing your ReachLink onboarding ensures we have everything needed for safe, effective, and compliant care. This process takes only a few minutes and must be completed before your first therapy session.

What You’ll Need to Provide

1. Basic Information

During onboarding, you’ll enter:
  • Full legal name
  • Date of birth and demographic details
  • Contact information (email and phone number)
  • Communication preferences (SMS, email, or both)
This information helps us contact you, match you with providers, and comply with healthcare requirements.

2. Emergency Contact

You must list at least one emergency contact.
This is required for your safety and is only used in urgent situations where immediate support is needed.

3. Medical Intake Form

The medical intake is a legally required clinical questionnaire that must be completed before your first session. It helps your provider understand:
  • Current symptoms
  • Medical and mental health history
  • Medications
  • Past treatment experiences
  • Risk and safety information
You cannot attend your first appointment until the medical intake is complete.

4. Insurance Information (If Applicable)

If you plan to use insurance, you’ll be asked to provide:
  • Insurance provider
  • Member ID
  • Group number (if applicable)
  • Photo of the front/back of your insurance card
Verification Timeline:
The Care Team typically verifies benefits within one hour.
You’ll be notified as soon as verification is complete.

5. Initial Clinical Assessments

Before your first appointment, you’ll complete:
  • PHQ-9: A 9-question assessment for depression
  • GAD-7: A 7-question assessment for anxiety
These assessments establish a baseline so your provider can track your progress throughout treatment.

After You Complete Onboarding

Once you finish all required steps:
  • You can browse available providers (filtered to your insurance network when applicable)
  • You may request or book your first appointment
  • Self-pay patients can confirm immediately
  • Insurance patients can confirm once verification is complete
  • You can add or update payment information

Important Notes

  • All information you provide is confidential and protected under HIPAA.
  • The medical intake and assessments must be completed before your first session.
  • Insurance clients cannot confirm appointments until benefits are verified.
  • Self-pay clients experience no waiting period and can confirm right away.
