- Your Care Coordination team will remind you as you approach the end of your EAP sessions
- You can continue with the same provider, if eligible, by transitioning to commercial insurance or self-pay afterward
Transitioning Beyond EAP
Before your EAP sessions end, ReachLink helps you plan the next steps to ensure uninterrupted care. You may contact your Care Coordinator (833-322-0977) at any time to verify your commercial insurance benefits early. This helps confirm that your transition to commercial coverage is secure well before your EAP authorization ends.
When you schedule your final EAP session, the Care Coordination team will also proactively reach out to discuss your transition options and ensure everything is in place for continued care.
Your options may include:
- Commercial insurance: Verified in advance for a seamless switch
- Self-pay: Available if you prefer or if no insurance is available
- Continuing with the same provider, when licensing and coverage allow
