Your provider may create a treatment plan:

How to Review and Sign
Notification
- You’ll receive a notification when a plan requires your review
- The alert will indicate that a plan is ready to be reviewed
Review Process
- Log into your ReachLink account
- Navigate to the treatment plan section
- Read the document thoroughly
- Ask your provider any questions during your session
Signing
- Digital signature is required
- Confirms that you have reviewed and understood the plan
- Becomes part of your official care record
- Signed plans remain accessible for future reference

By signing your treatment plan, you:
- Confirm you have reviewed the plan
- Indicate agreement with the proposed treatment direction
