How to access your clinical notes and what to expect
ReachLink is committed to transparency and client rights. You have a legal right to request and receive a copy of your session notes. This guide explains how to submit a request, what to expect, and how long the process typically takes.
How to Request Your Session Notes
Session note requests should be submitted through the Get Help feature in your ReachLink account.
How to Begin
- Log into your ReachLink account.
- Select Get Help from the menu.
- Submit a ticket requesting your session notes.
- Include:
- The specific dates or date range you need
- Whether you want all available notes or only selected sessions
- Any deadlines or additional context (optional)
Your request will be routed to your provider for review and completion.
What You Can Expect
After your request is submitted:
- Your provider will be notified
- They will prepare the appropriate clinical notes for the requested dates
- You will receive the notes once they are completed and uploaded
Session notes typically include:
- The date of each session
- A clinical summary
- Relevant observations or assessments
- Updates to your treatment plan (when applicable)
Note: These are clinical notes—not transcripts—and are written for clinical documentation purposes.
How Long It Takes
Most session note requests are completed within:
5–7 business days
More extensive or complex requests may take additional time. If you have a deadline, please include it in your ticket so we can try to accommodate.
Your Rights
You have a legal right to access your own clinical notes. ReachLink will honor all valid requests unless releasing the notes would pose a safety concern (rare).
Additional considerations:
- Only your own notes can be released
- Couples or family notes may require consent from all participants
- Providers cannot alter or edit old documentation

How You’ll Receive Your Notes
Once completed, your session notes will be uploaded to your account.
Where to Find Them
- Log into your ReachLink account
- Go to My Account
- Open the Uploads section
- Download your session notes from there
